Filling out your paperwork prior to your first visit will save you time and give your therapist time to review your case prior to your appointment. Please fill out the online paperwork that was emailed to you shortly after you scheduled your first visit. If you have trouble with this, please print and complete the paperwork below prior to your first appointment.
If you choose to wait until your visit, please arrive at least 15 minutes prior to your first appointment.
Please note that due to our downtown location, we do not have a designated parking lot. Free two hour spots are available a block away, and we have quarters inside our office for you to feed your meter if you park on-street out front.
